Prepare the following list of documents, including:
- Document confirming the state registration of the legal entity
Note: An extract from the relevant business register (commercial, banking, or other register in which the state registration of the company or organization is recorded). - Document confirming the authority to act on behalf of the legal entity
Note: Appointment order, contract, or other document in accordance with the legislation of the country of registration. - Identity document (passport or ID card).
- Document confirming the identification number (tax or personal registration number).
Note: All listed documents must be provided with a notarized translation into Ukrainian (or its official equivalent in accordance with the legislation of the respective country, such as a sworn, court-certified, or consular translation).

Prepare the following list of documents, including:
- Identity document
Note: This may be: a passport of a citizen of Ukraine, an ID card, or a passport for travel abroad. - Taxpayer card (tax identification number – RNOKPP)
Note: This may be a Certificate of Assignment of the Identification Code, or an RNOKPP mark in the identity document.
Note: For individuals engaged in independent professional activity, at the request of the customer, additional information may be entered into the qualified certificate (for example, the certificate number for the right to practice notary activity, etc.). In such cases, the original supporting document must additionally be provided.

Prepare the following list of documents, including:
- Constitutional documents
Note: This may be: a founding agreement, a decision to establish a legal entity acting on the basis of a model charter, or its notarized copy (provided solely for review). In case of amendments to the charter or the creation of a legal entity after January 1, 2016, an inventory document (opis) with a note containing the access code to the portal of electronic services for legal entities, individual entrepreneurs, and public formations must be submitted (solely for review). - Document confirming the signatory's affiliation with the legal entity and their authority (original or certified copies)
Note: This may be: an appointment order, an extract from the appointment order, or an employment contract. - Identity document
Note: This may be: a passport of a citizen of Ukraine, an ID card, or a passport for travel abroad. - Taxpayer card (tax identification number – RNOKPP)
- Note: This may be a Certificate of Assignment of the Identification Code, or an RNOKPP mark in the identity document.
Note: If the service is received by an employee, an application of the established form signed by the head or another authorized representative of the legal entity must be submitted. If such a representative acts on the basis of a power of attorney (and their details are not listed in the EDR), this power of attorney must also be provided.
Completion
Personally visit the registration administrator with the required set of documents to undergo personal identification.

Other requirements:
- Ensure the ability to receive technical messages to a phone number via the Viber messenger (for phone numbers from the following countries: Poland, Germany, Czech Republic, Belgium, Slovakia, Spain, Italy, United Kingdom, Bulgaria, Canada, Moldova, Slovenia, USA, Greece, Estonia, Norway, UAE);
- OR use an eSIM of a Ukrainian operator (+380...).
